Check the following:
1. Connector on the target cable has a tendency to get loose if plugged/unplugged frequently. Make sure the connection is good and replace the cable if needed.
2. The target connector to processor distance on the target board should be less than 6 inches.
3. Make sure the clock is not being overloaded by other devices on the board.
