New Product Release Forms

These release forms are used to track and document all the steps involved in implementing a new speech code onto one of the parent speech devices. Blank forms are available below, or get the appropriate forms from your TI sales office to initiate the process.

Section 1 of the forms allow you to choose the parent device for your particular code, as well as the options pertinent to the parent device. You can also choose your own customer part number used for ordering your parts. If no customer part number is indicated, a TI default part number will be issued for ordering purposes. Completion of the company name, project name, and option fields is mandatory. Other fields in Section 1 are optional. Completed forms (along with your speech code) must be submitted according to the directions on the form through your local TI sales office.
